Your First Appointment
To ensure your best quality care, your medical and surgical history is very important. It is helpful to bring in a list of current medications including doses with you to every appointment. It would be helpful to have copies of any x-rays, CT scans, or MRIs that are related to the problem for which you are being seen. The name and contact information of your primary doctor (as well as any necessary referrals) allows for more efficient exchange of information between our office and that of your primary care doctor.
We accept coverage from many different insurance companies. Please call your insurance company to confirm our participation in your plan.
